External Providers

External Providers

Please note, at Chatham Public School: 

  • there is no external provider support during Term 1, and
  • all external provider support is to occur in Terms 2, 3 and 4 between 2pm and 2.45pm.

To engage external provider support at Chatham Public School:

Step 1

Once the parent/carer has connected with the external provider, they are to complete the Parent/Carer Request Form and return it to the school office. A hard copy of this form can be collected from the school office if unable to print form here. 

Step 2

The Learning Support Team at Chatham Public School will discuss the application. Parent/carers and external providers will be informed  via an email of the decision (approved or not approved).

If the request has been approved, Chatham Public School will then contact the external provider where they will then be required to provide the following:

  1. An external provider agreement
  2. Current child protection training (to be completed annually)
  3. Declaration for child related workers

External Providers must read and be aware of:

  1. Code of Conduct Policy
  2. Controversial Issue in Schools Policies and Procedures
  3. Allegations Against Employees in the Area of Child Protection

External Providers must provide evidence to Chatham Public School of:

  1. Public Liability for at least $20million
  2. Personal Indemnity for at least $2million
  3. Workers Compensation at Insurance
  4. Completed Child Protection Training for the current year
  5. Photo ID (eg employee ID or drivers license). 

Step 3

The Chatham Public School Learning Support Team will contact the external provider to inform that services can commence.

Please Note:

External provider services are not to commence until the above process is complete.
